1. Print the pre-application, sign it and fax it to (407) 293-2968. You will then be contacted by a staff member to complete the pre-application process.
   
   
2. Then you will be asked to print out the Client Application and the Program Rules, which you will also fill out, sign and fax to (407) 293-2968.
